Benefits are offered to full-time employees working 30 hours per week or more at John C. Fremont Healthcare District. Employees are eligible for the benefits the first of the month following 30 days and must be accepted or declined upon eligibility. Medical Insurance - Employees have a choice of many different health insurance plans - a Point of Service plan (POS) or HMO Plan. These plans are administered through Anthem Blue Cross. Under the HMO, all of your care is coordinated by a primary care physician with most services covered at 100% after a copayment. Under the POS plan, there are two tiers of coverage to choose from. Your out-of-pocket costs will vary based on the tier you use. Premiums vary based on the plan selected.
Vision Plan - This plan covers routine eye exams and vision hardware (i.e., frames, lenses, and contact lenses).
Dental Insurance - The dental plan features a network of more than 500 dentists. The plan allows the option of seeing a network dentist or obtaining care through another dental provider.
Additional Life may be elected for spouse and children at an additional cost.

PTO is calculated based on hours worked at .0925. PT is entitled to 50% of the annual accrual and cap. (Annual cap does apply).
LTS - maximum accrual 160 hrs.
Sick - PD and PT eligible. PD-3 days annually after 90 days of employment, PT 1 day annually after 90 days employment.

Retirement 403(b)/457 Plan
Flexible Spending Account (FSA) Available the first of the month following 30 days of continuous service in a benefit-eligible status. These employees are eligible to participate in the Flexible Spending Account program for healthcare expenses or qualified daycare reimbursement. Through payroll deduction on a pre-tax basis, employees may utilize this plan to set aside money for out-of-pocket healthcare expenses, dependent care provider costs, or both.
